INSURANCE PAYMENTS

Nancy is a preferred provider for most health, medical, motor vehicle Personal Injury Protection claims, and Labor and Industries Insurance programs in Washington. Contact your insurance provider to confirm Nancy Haller, Massage Therapist and GCFP is a preferred provider. Understand your applicable deductions, copays and requirements of a Massage Therapy Referral for medically necessary sessions.


INSURANCE POLICIES

With all the current changes to health care coverage we are providing you the details regarding the process necessary for receiving insurance billed services from Nancy Haller.

Practitioner Responsibility (Nancy Haller)

  • To notify the insurance company of the client's (you) session with the practitioner.


  • Submit proper timely billing paperwork to the insurance company.


  • Practitioners must maintain records.


  • Send Records, with client approval, as requested to insurance companies, physicians, and attorneys.

Client Responsibility (You)

You and/or the insurance company are responsible for payment of all sessions.


  • Your insurance policy is a contract between you and your insurance company.


  • Understand your personal policy benefits, deductibles, and co-payments.


  • Bring your current insurance card and physician referral (if necessary) to your first appointment.


  • Notify your practitioner if you have any changes in your insurance coverage.


  • Fill out the Advanced Wellness Connection registration form completely for your personal health record and ease of billing. The forms are a portion of your health records to be maintained by the Practitioner.


  • Maintain current personal information, address and phone number with our office.


  • Make immediate contact with your insurance company and Practitioner when an Explanation of Benefits (EOB) does not make payment to the practitioner.


  • Make payments to practitioners for all copays and deductible sessions not paid by your insurance company in a timely manner.


  • VISA, MC, check and cash are accepted.
